Record of Daily Work Assignments

Our apprentice program is approved by the United States Department of Labor, and we are required by law to keep records on the work of each apprentice. To comply with federal law, this report must be submitted promptly.

This report must be submitted each month - whether working or not!

Each apprentice shall give an accounting of his or her daily work assignments under the proper headings, accounting for all hours worked. Columns will automaticall total. Once you have completed the report, click the "Submit" button at the bottom of the page to send the report. This report must be submitted by the 10th day of the following month.
